Notification of Your Gift

If you would like, the Foundation will send a personalized acknowledgement to the individual or family you are honoring.

The acknowledgement will notify them that a gift has been made in honor or memory of their loved one without disclosing the amount of your donation.

During your online donation, simply indicate the name of the individual you wish to honor or remember and provide the contact information for the person or family who should receive the acknowledgement.

You may choose to direct your gift to:

• Village Center Campaign
• Pediatric Comfort Fund
• Medical Equipment & Technology
• Community Health Programs
• Another Foundation fund of your choice

If no designation is selected, your gift will be directed to the Foundation's Greatest Need Fund, allowing support where it is needed most.
